Эффективность шинирования зубов у людей страдающих сахарным диабетом.

 Организм человека представляет собой единое целое, что обуславливает зависимость тяжести и распространённости стоматологической патологии от состояния всего организма. В современных условиях наблюдается рост хронических заболеваний общесоматического профиля, что связано с фоновыми условиями обитания человека, которые значительно ухудшились и осложнились в силу нерациональности жизнедеятельности самого человека. В нашем исследовании приняли участие 49 пациентов с диагностированным сахарным диабетом 2-го типа и 49 пациентов контрольной группы не страдающих сахарным диабетом. Целью исследования была оценка влияния шинирования зубов на пародонтальный статус пациентов страдающих сахарным диабетом 2-го типа. Проведенные исследования показали, что у лиц, не страдающих сахарным диабетом 2-го типа подвижность зубов, после проведенного шинирования, уменьшалась в 100% случаев, а у людей страдающих сахарным диабетом 2-го типа уменьшение  подвижности наблюдалось несколько реже.

 The human body is a whole, which leads to dependence severity and prevalence of dental disease on the state of the whole organism. In modern conditions the rise of chronic diseases somatic profile that is associated with the background conditions of human life, which have deteriorated significantly and complicated due to the non-rationality of life of the individual. In our study involved 49 patients diagnosed with Type 2 diabetes and 49 control patients not suffering from diabetes. The aim of the study was to evaluate the effect of splinting teeth on periodontal status of patients suffering from Type 2 diabetes. Studies have shown that individuals do not suffer from Type 2 diabetes tooth mobility, conducted after splinting, decreased in 100% of cases, and in people with Type 2 diabetes decreased motility were observed less frequently.

Введение

Somatic pathology and iatrogenic intervention affect the character of the flow of periodontal disease. The incidence of periodontal disease in people with diabetes varies 80-90% [1, 2]. It is known that Type 2 diabetes (T2D) affects the periodontal tissues, reducing their reactivity to the effects of environmental factors, worsening the condition of periodontal tissues at the cellular level, causing microcirculatory disorders. According to research by Genco RJ (2010) it was found that under the influence of bacteria, diabetes and inflammation occurs secretion of inflammatory mediators under their action observed activation of osteoclasts, leading to bone resorption and decrease in activity of osteoblasts responsible for bone formation. [3] These processes are no longer cancel each other out, and destructive processes prevail in the periodontium lead in accelerating the destruction of the jawbone. Also, all these processes take place against the background of reduction of local immunity. In this regard, the effectiveness of treatment and rehabilitation measures in periodontal diseases with type 2 diabetes will be reduced.

Evaluate the effectiveness of splinting teeth in individuals with diabetes.

Материал и методы

We examined 49 patients suffering from type 2 diabetes who have turned to a periodontist. As a control group (C) participated in the study and 49 patients referred to a periodontist. The patients' age 30-45 years. In 15 patients with type 2 diabetes and 15 of the C was moderately severe periodontitis. In 10 patients with type 2 diabetes and 10 of C, was mild periodontitis severity. In 12 patients with type 2 diabetes and 12 of C, was 1 degree periodontal disease. In 12 patients with type 2 diabetes and 12 of the C 2 degrees with periodontal disease (Table 1). All patients who participated in the study, there was a pathological tooth mobility grade 1 or 2 (Table. 2) .In the complex therapeutic measures to reduce the mobility of the teeth, teeth splinting conducted fiberglass thread. To register changes patients arrived for a visit in 9 months. After removing the tires had emerged accounting changes.

Результаты

In the group of patients with mild periodontitis severity with type 2 diabetes was observed: in 4 patients (80%), tooth mobility decreased from 2 degrees to 1 degree; 3 patients (60%), abnormal mobility of teeth 1 largely disappeared. In the control group of patients with mild periodontitis severity was observed: in 2 patients (100%), tooth mobility decreased from 2 degrees to 1 degree; 8 patients (100%) pathological tooth mobility degree 1 is absent. (Table. 3) In the group of patients with moderately severe periodontitis with type 2 diabetes was observed: 10 patients (71%), tooth mobility decreased from 2 degrees to 1 degree; 1 patient (100%) pathological tooth mobility 1 largely disappeared. In the control group of patients with moderately severe periodontitis was observed: 10 patients with tooth mobility decreased 2 degrees at 1 degree and 2 patients pathological tooth mobility disappeared (100%); 3 patients (100%) 1 pathologic tooth mobility largely absent. (Table. 3) In the group of patients with periodontal disease 1 degree with type 2 diabetes was observed: 10 patients (83%), abnormal mobility of teeth 1 largely disappeared. In the control group of patients with periodontal disease 1 degree abnormal mobility of teeth missing (100%). (Table. 3) In the group of patients with periodontal disease 2 degrees with type 2 diabetes was observed: in 2 patients (100%), tooth mobility decreased from 2 degrees to 1 degree; 8 patients (80%), abnormal mobility of teeth 1 largely disappeared. In the control group of patients with periodontal disease grade 2 there was a lack of pathological tooth mobility (100%). (Table. 3)

Обсуждение

Efficacy splinting of teeth, parodontitis patients suffering from mild severity of diabetes-2 was 70%, and in patients with moderately severe periodontitis with DM-2, and is slightly higher than 73%, while in the control group decreased mobility in 100% . A similar trend was observed in patients with periodontal disease. The effectiveness of splinting teeth in patients with periodontal disease 1 degree with DM-2 is 83%. In patients with periodontal disease grade 2 with DM-2 efficacy of splinting teeth corresponded to 71%. While in the control group, the clinical effect was observed in 100% of cases.

Заключение

The findings suggest that splinting teeth is an important component of the treatment of periodontal disease in patients with diabetes mellitus.
